After weeks of intense negotiations between the HSE, the Department of Health, and health workers’ unions, an agreement has been reached on the HSE Principles for Extended Services. The initiative aims to enhance patient access to services by extending opening hours, while ensuring staff protections are underpinned by a structured implementation plan in line with public service agreement commitments.
Unions secured eight key protections (outlined below) for members as part of the agreement, which will see healthcare services operating over extended hours in a managed and sustainable way. The agreement includes provisions to protect staff welfare, ensure safe working conditions, and provide fair compensation for those participating in extended rosters.
SIPTU Health Divisional Organiser, Kevin Figgis, said: “The agreement marks a significant step forward in reshaping Ireland’s healthcare services while balancing the needs of patients and staff. By ensuring robust protections and structured implementation, the HSE and the unions aim to deliver extended services sustainably, without compromising worker welfare or patient safety.”
The extended services initiative is expected to be rolled out in the coming months, with unions and management continuing to engage on local implementation plans.
- Adherence to Existing Agreements
The HSE has committed to upholding the principles of HSE HR Circular 003/2009 and the updated Framework Agreement 2008. The agreement also ensures that existing Labour Court recommendations on extended working days (such as those for radiographers and radiation therapy) remain unchanged.
- A Safe Environment for Staff and Patients
Any extension of services must guarantee a safe environment for both staff and patients. Proposals must not lead to existing services being delivered with inadequate or unsafe staffing levels.
- Management Oversight and Governance
Extended services must be supported by appropriate management oversight. Local management will be required to submit detailed plans for extending service hours beyond current working patterns.
- Volunteering and Eligibility
Staff will initially be asked to volunteer for extended hours. If insufficient volunteers come forward, eligible staff (those employed or promoted after 16th December 2008) will be rostered based on criteria set out in HSE HR Circular 003/2009.
- Dispute Resolution Process
A clearly defined dispute resolution process has been agreed, involving:
Stage 1: Local discussions
Stage 2: Joint Review Group (with staff and management representatives)
Final Stage: Independent third-party adjudication
- Compensation for Loss of Earnings
Where extended hours result in lost earnings, affected staff will be compensated under the recognised Public Service Agreement formula.
- Premium Payments for Extended Hours
Staff in roles not traditionally eligible for premium payments will receive them when rostered for extended service periods.
- Regular Review of Extended Services
The new arrangements will be reviewed after one, two and three years, assessing effectiveness, user satisfaction, costs, and ongoing need.
